Most documents are downloadable and must be submitted in person on the first day of school.
• Parkville Campus students must bring forms to their homeroom teachers.
• Commonwealth Campus students must bring forms to the High School Office.
Some essential documents included in the Annual Agreement must be completed digitally through our Student Information System (SIS) portal. These include:

2 Student Handbook Agreement (Parkville & Commonwealth)
3 Student Handbook Agreement (Parkville & Commonwealth)
4 Walker’s Consent Form (Parkville Only)
5 Shuttle Bus Form (Commonwealth Only)
