Opt-Out/Withdrawal Form

By completing this form, Commonwealth-Parkville School (CPS) will be notified that one or more of the children in your family will not be returning to school for the academic year 2025-2026. Therefore, they will be withdrawn and their place at CPS can no longer be guaranteed.
 
The opt-out/withdrawal period runs from February 1 through February 14, 2025. If you wish to withdraw one or more of your children for the 2025-2026 academic school year, please submit this form no later than February 14th.

Please note that the Opt-Out Form information is only used for billing and admissions purposes, and is not shared with the registrar. If you need your transcripts forwarded to another school, please contact our registrar Lizbeth Salgado at lsalgado@cpspr.org.
